After School Care Weekly Reservation Form
Please submit this form to the office to reserve after school care.

Field Trip Permission Form
Each teacher provides a permission form for each field trip that details specific information. You may use this general Field Trip Permission Form provided by the Catholic Schools Office, fill in all the information that you can (especially your signature and the phone number of where you can be reached during the field trip), and return the form to the teacher as a substitute permission form.

Manna is a program designed to offer parents a means of offsetting the cost of school tuition or of donating their profit to the school, other families, or teachers for school-related expenses. Use these forms to register in the program and to order certificates. The order and your check are due in the school office or Rectory by Monday at 8:00 a.m. Orders are filled by Tuesday afternoon and sent home in the family envelope or are left in the school office or Rectory for  pick-up. Parent/Student Handbook Waivers
Review, date and sign these waivers and return to the the school office to certify that

1)  The household parents and children have read and discussed the information contained in the Parent/Student Handbook and and that you agree to be governed by it.

2)  You have designated contact information in the event of an emergency.

3)  You have read and reviewed the photograph/video/website waiver and signed it if you prefer your child(ren) NOT to be in any photographs, videos to be used for school publications, publicity and the St. Mary of Gostyn website.

High School Information Request Opt-Out Form
Review, date, and sign this waiver if you wish for information about your student in grades 6, 7, 8 not to be shared with high schools requesting this information.

MEDICAL FORMS

Food Allergy Action Plan
Submit this form to your physician so he/she can indicate what must be done for students with serious food allergies that must be controlled by antihistamine or epinephrine.

Health Exam Form
Submit this form to your health care provider so he/she can certify that your child has had the required physical examinations and immunizations. (See Health Concerns in the Parent/Student Handbook (page 16) for further information.)

Dental Exam Form
Submit this form to your dentist so he/she can record information from your child's dental examination. This exam is required of all Kindergarten, grade two, and grade six students.

Vision Exam Form
Submit this form to your eye doctor or optometrist so he/she can record information from your child's eye examination. This exam is required of all Kindergarten students.

Sports Physical Form
Submit this form to your physician so he/she can certify that your child has had the required sports physical examination needed to participate in our Athletic Program.

Student Administration of Medication Forms
All medication is self-administered by the student under the supervision of the school personnel. A student must have these two forms on file in the office. (See Health Concerns in General Information for further information.)

VOLUNTEER AUTHORIZATION & PROCESS
