June 2016:  Sports Physicals

If your son or daughter has signed up for volleyball and/or cross country and we will need to have a valid physical form on file for them.  Each SMG student-athlete is required to have a sports physical completed annually.  This form is signed by a physician and gives medical clearance for the student to participate in athletics for one year.  This form needs to be completed and handed into the school office  by no later than August 1st in order for your son/daughter to be eligible to participate in volleyball and/or cross country. If you had a sports physical completed last year after August 1st, it is valid for a full year but it will need to be renewed immediately following its expiration. Your son/daughter will not be allowed to participate until this form has been handed in to the school office.

For 6th graders, the comprehensive school physical will suffice.   For all other grades, the sports physical form needs to be completed and submitted.

If you have any questions or comments about your child's sports physical, please send them to
